I don't have a copy of it because, at that time i was living at her home. So i gave my grandma temporary guardianship of my son and got it notarized on Feb. I ended up relapsing an used for about 2 weeks. my son is 11 months old and I also have an open voluntary family maintenance cps case in the state of California.
